1. Customer will provide the following information:
A. Complete name and address of shipper and receiver.
B. Phone number of shipper and receiver.
C. Contacts at shipper and receiver.
D. Shipping and receiving hours.
E. Type of equipment required.
F. Weight and number of pallets.
G. Commodity.
H. Presence of hazardous materials.
I. Payor of freight.
K. Credit references if payor of freight.
2. Customer will tender bill of lading to carrier.
3. Customer will pay invoice upon receipt.
4. Customer is responsible for filing all claims with carrier. A copy of claim to be forwarded to Time $aver Transportation, Inc.®, who will assist, if requested, in the claim process.
5. Shipper / Broker Agreement is available upon request. Not a requirement of Time $aver Transportation, Inc.®
